Referring yourself or a family member to our service
Sometimes you may not know which clinicians may be best suited for you or your family member. By completing the relevant Intake Form below, we can go through the information you have provided and give you some options as to which clinician may be the most suitable for you.
There are different Intake forms depending on the age of the person wanting to access our service and depending on whether they are NDIS funded or not. Please read through the options carefully to select the relevant Intake Form.

Online Appointment Booking System
If you know which clinician you would like to book appointments with and they are shown below, you can click on the "Click here to book" buttons under their names to book an appointment straight away. Once you have booked the appointment, you will be emailed an intake form to complete prior to your first appointment.
If you would like to use NDIS funding for your appointments, please do not use the online appointment booking system at the moment. Please complete the relevant Intake Forms above and we will get in contact with you to arrange your first appointments.
Not all clinicians are able to be booked through our online appointment booking system for a variety of reasons including: limited availability, changing working capacity or varying days of work.
Please note: When booking through the online appointment booking system, the appointments will be for an initial intake appointment. As part of this appointment, the clinician will work together with you to determine if they are the right fit clinician for you or if someone else at our service may be better suited to you for further appointments.
If you book an appointment through our online booking system, the location of the appointment will already be set. If you need to change the location/format of the appointment (in person, online videoconference or over the phone) please contact our office on (03)54 823 143 after you have booked online, and we can review the schedules for you and let you know.
